Here is the exact description of the plugin which I was searching. The survey plugin almost satisfies my requirments.
I need a plugin to generate forms. The details will of the forms which are filled will go into the forum. The 3d party will view the post in the forum and will try to answer to the forum and the reply will be stored as well as emailed to the person who had posted in the forum. I am also looking for uploading videos and photos in those forms.

I downloaded the 0.4.8 survey and tried to create the survey, but the link which is generated is not working properly and i get a page not found error. Can you please help me on this?

Get the latest version of the plugin (still found here).

It will be the top most link (as of this post the current version is 0.4.10). Due to a file structure change, you'll need to remove all the files inside the survey folder before uploading the most recent version.
