Hello,

I am looking for a plugin that would allow me to:

1. Have a form to collect some basic information such as name, email, phone, company.
2. Send an email with a link to a file that can be downloaded.

Is there an existing plugin that would do this? If not, I need this rather quickly.

Survey Plugin, it can post a form to a forum, e mail it to you etc.

Thank you both for the response. I could not find the survey plugin on this site, but did manage to find it on the e107coders site. Perhaps I need to clarify my requirements:

I would like users to fill out a form requesting a file. They submit their information such as name, company, phone, and email. In turn the form would send an email to the person who submitted the form with a link to the file that is being requested.

As far as im aware the survey plugin sends an email to the user upon submission.
If thats the case then you could include the link within that email.
But that is not perfect if you have multiple files / links.

The other option would be just to include your link within the After Submission page within the survey plugin, that way it excludes email and presents the link instantly.

The only draw back is the file url will always be static and could be open to abuse if you wanted your urls unique to each user.
